Post School
Flexible lifestyle enhancement choices
In the provision of post school services, Breakaway Incorporated aims to plan and provide flexible, individualised activities and opportunities for individuals with complex and high support needs.

Three Key areas that are vital to the participation in society for individuals with a disability:
  1. Growth (Emotional and Intellectual)
    • Primary focus: - to assist our guests to achieve goals to enhance their emotional and social wellbeing. Opportunities to gain skill and confidence that are needed to be part of the community. Support provided in an environment that is empowering, and maintains dignity and respect.
  2. Movement:- (this includes Transport)
    • Access to building that serves public- When planning outings. Suitability of venues and transport/personal care options are always researched prior.
  3. Location:- (increasing opportunities to be active citizens
    • Our aim is to give our guests opportunities to access and experience what the community has to offer as well as becoming active participants.
Breakaway Inc aims to interrelate these three areas to give our guests the opportunities to experience growth, development and individuality, and a quality of Life, as members of the wider community.

Breakaway Post School Lifestyle program:

•  Caters for Young adults with high and complex needs
•  The program operates for 48 weeks per year and offers one on one support to enable participation and integration into community life.
•  Our Post School premises at Balmoral Street, is set up for art/craft/music appreciation as well as a drop in for individual projects and rainy day activities.
•  Flexible and responsive to the individual and family needs, working in partnership with parents and family member in planning evaluation and decision making process;
•  Participation (opportunities for strong community support).
•  Further develop skill and abilities, which may include a passion for music/movement; self help/personal care abilities; social interaction; friendship.
•  Opportunities to gain sense of self and others.
•  To assist the general public to become more aware and responsive to people with a disability.
•  Greater access in community based activities.
•  Safe, supportive, adult supervised environments that offer various growth-enhancing opportunities, activities and experiences.
•  Belonging - by building social networks and links with other organizations and the community

The goals of Post School Lifestyle Program:
Our goal is to provide individuals with a disability the same choices and experiences as members of the wider community. Young people benefit when they spend time engaged in structured pursuits that offer opportunities for positive interactions with adults and peers.
•  Encourage to contribute and take initiative.
•  Give opportunities for challenging and engaging tasks that help develop new skills and personal talents
Through opportunities that engage meaningful age-appropiate activities;

•  Social relationships with peers without disabilities;
•  Personal growth and development, individuality, companionship and friendship.
•  To work in partnership with parents and family
•  Having support workers that are companions that work together as a team - thus giving full support to our guests.
•  Most importantly BREAKAWAY INCORPORATED aims to enhance the person's QUALITY OF LIFE through diverse life experiences and choices.

Overnight Support
Overnight support is provided in our residential house located at Margate.

The Breakaway House has:
  • 4 guest bedrooms, each with fully adjustable beds;
  • 2 x Guest bathrooms;
  • Lounge and family room;
  • Outside area with bbq; and
  • 7 seater outdoor heated spa;
Currently, Breakaway supports 35 families, providing overnight respite for a minimum of 34 nights per year per child, including school and public holidays and weekends.

The support provided is in an environment that is least restrictive, values self-worth and respect and is indicative of and responsive to each guest's individual needs including all aspects of personal care, health and well-being, recreation activities and choice.
 
Emergency Support
Breakaway supports families and guests in crisis by providing emergency care and support dependant on family circumstances. Overnight emergency respite can be accessed at Breakaway as is needed by member families or through referrals from the Department of Communities, Disability Services Queensland and/or other service providers. In the 2003-2004 periods, Breakaway offered emergency overnight support to 25 families in crisis.

In addition, Breakaway works closely with other community service providers in gaining funds to provide emergency support in the family home as is required.
